One of the key elements of the Final Regulations is the new provision that requires facilities to conduct and document a facility-wide assessment to determine what resources are necessary to care for its residents competently during day-to-day operations and emergencies. The facility assessment must address the facility’s resident population (number of residents, types of care and staff competencies required by the residents and cultural aspects) and resources (equipment and overall personnel) and a facility-based and community-based risk assessment.

The facility-wide assessment and an initial QAPI plan are part of Phase 2 implementation due by November 28, 2017. The facility assessment and QAPI plan are interconnected. The facility assessment defines the services provided, needed staff competencies and staffing requirements. Failures in quality may be linked to and analyzed in relation to the facility assessment and the QAPI plan.

  • List resources available to facilities to assist with the facility assessment.
  • Explain responses to frequently asked questions related to the facility assessment process.

MEET THE PRESENTER
Kathleen Weissberg, OTD, OTR/L, Education Director, Select Rehabilitation
Kathleen Weissberg, in her more than 20 years of practice, has worked in long term care as a researcher/educator and has established various programs in nursing facilities including palliative care and end of life care, incontinence management, falls management and dementia care and staging. She currently provides continuing education support to 6,000 therapists nationwide as director of education for Select Rehabilitation. She has lectured nationally, at the state level, and authored publication on the topics such as patient satisfaction outcomes in long term care, restraint reduction and fall prevention, driving and the elderly, dementia management, palliative care, and hospital readmissions. She has presented at previous LeadingAge conferences for Georgia, South Carolina, Michigan, New York, and has authored articles for publication for LeadingAge Florida and Texas.
